To Schedule an Appointment
Give us a call at 425-391-4488 and we will schedule you with the therapist that will best serve your needs. Please have your insurance card in front of you so we can ask you for some information regarding your insurance coverage. Our appointment desk hours are Monday - Friday 8:00 am to 5:00 pm. We ask that you give us a 24-hour notice of cancellation so that we may best meet the needs of all of our patients.
Your First Visit
When you come to our office, you will need to bring the following:
● Patient registration forms. The necessary forms are located under the Patient Forms tab. If you are unable to print these forms we can fax them to you or you can come in 15 minutes before your appointment to complete the forms (we are unable to email them to you).
● Your Insurance card or cards and Driver's License/ Photo ID
● Your prescription or referral from your doctor (if required).
● Any applicable copay. We accept check, cash, Visa or MasterCard.
Your therapist will perform an evaluation and begin treatment. You can expect the first visit to last approximately one hour. The results of the evaluation, your goals and treatment indications will be discussed thoroughly. You can expect to receive a series of exercises to perform at home to speed recovery and allow for effective rehabilitation.
We are contracted with the following Health Insurance Companies:
Aetna, AARP, Cigna, Department of Labor & Industries, First Choice, Group Health, Great West, Lifewise, Medicare, Motor Vehicle PIP, Pacificare, Premera Blue Cross, Regence Blue Shield, Secure Horizons, Tricare, United Healthcare and Uniform Medical. For patients without insurance coverage we offer Private Payment Plans. Please contact our clinic if you have any questions regarding insurance or billing.
We will review your benefits with you on your first visit.
